This article provides step-by-step instructions for creating an exit survey template, including sample questions and some time-saving tips.

An Exit Survey allows a departing employee the opportunity to comment on their reason for leaving your company. They can be helpful for obtaining feedback that can help you improve aspects of your organization. Follow the steps detailed below to create our Recommended Exit Survey Template.

SURVEY TEMPLATES

Customize Template Names

  1. Click Create New Template.

  2. Enter the title of the New Review Template. Here are some examples:

  • Exit Survey

  • Exit Interview Questionnaire

  • Employee Exit Survey

Add Questions

  1. Click Add Section > Name the section title as Leadership.

  2. Select Questions Include: None.

  3. Click Add Questions > Add the below question and answers.

  4. Select the Answer Type: Rating (on a list of many)

  • 1. Never

  • 2. Sometimes

  • 3. Almost Always

  1. Select the checkbox for Hide Numbers and Allow employee to comment on the rating > Done.

  1. Click Add Section to add another section > Name the section title as Duties and Responsibilities.

  2. Select Questions Include: None.

  3. Click Add Questions > Add the below question and answers.

  4. Select the Answer Type: Rating (on a list of many)

  • 1. Poor

  • 2. Fair

  • 3. Good

  • 4. Excellent

  1. Select the checkbox for Hide Numbers and Allow employee to comment on the rating > Done.

  1. Click Add Section to add another section > Name the section title as Open Questions.

  2. Select Questions Include: None.

  3. Click Add Questions > Add the below question and answers > Done.

  1. Click Save and Publish.

TIPS

The following checkbox selections can be configured to your questions:

  • Selecting the Add N/A as an option checkbox can be useful if a question might not be answerable by the employee.

  • Selecting the Hide numbers checkbox can be helpful if you do not want employees to focus on the numeric value assigned to a rating.

  • Selecting the Allow employee to comment on the rating allows the departing employee to add additional feedback on questions with ratings as answers.
