Accessing 1095-C Forms

Provides instructions on locating Form 1095-C in Namely.

OVERVIEW

You will receive a 1095-C form from your employer if you work for an organization that has fifty or more Full-Time Equivalent employees. This form contains information about the health coverage offered to you over the course of the calendar year. You may need this form to complete your personal tax filings. The IRS requires that this form is provided to you by March 2, 2023, if applicable.   

ACCESSING YOUR 1095-C

You can access your 1095-C forms directly in Namely by following these steps:

1. Go to your Profile and select the Payroll tab.

2. Click Year End Documents.

3. Select your 1095-C.
