What is Fed Proof?

941 dated within the last year – complete form, not just page 1, must be signed

Copy of EIN Verification letter from IRS; client can call 800.829.4933 and request a
147c letter be faxed to them -
Any letter from the IRS received within the last year, as long as it shows name,
address and FEIN -
Signed business/corporate return (1120, 720, 1065, etc.) from the previous tax
year. Can accept forms from two years prior if:-
Filing deadline for previous tax year has not passed, or
-
If client has filed for an extension for the previous tax year. In this case,
copy of filed extension will also be required

Pre-printed 8109 coupon with IRS bar coding

Signed & dated copy of SS-4 application

Must have the FEIN on the form

If application was done online, client will need to print application and either sign and date, or include the online confirmation page

Statement of Deposits and Filings – clients coming off of ADP

Tax Recap/Quarterly Summary report from other vendors
What is rejected for Fed Proof?

Incomplete or unsigned 941
-
941 indicates no further wages will be paid
-
Proof is more than one year old, except in the circumstances indicated above

Multiple proofs provided with different names

Items are crossed out on form, major corrections
